There are two forms that need to be submitted every trimester. The Service Group Goal Sheet will be due near the beginning of each trimester and only one of these sheets needs to be submitted per group. The Progress Report will be due at the end of each trimester and every member needs to submit their own copy.

Both of these forms need to be turned in to the Equality Office on the 4th floor.
